Rock Island High School's Athletic Department will be hosting free Sports Physicals on Wednesday, July 31st from 6:30 to 8:00 pm. This is for 7th and 8th-grade athletes as well as high school athletes. Please note the doors close at 7:45 pm. 

 

Athletes will need to have the IHSA Pre-Participation Evaluation Form (form attached below) filled out. The FRONT page (Medical History) needs to be completed before arrival. The form must be signed and dated by a parent/guardian. 

 

When: July 31st

Time: 6:30 pm-8:00 pm (Doors Close at 7:45)

Where: Athletic Office, Nurse's Clinic, Island (Use Main Entrance to check in)

Who: High School Athletes, 7th & 8th Grade Athletes

Info: Medical History MUST be completed. Form MUST be signed and dated by Parent/Guardian (physical is not valid without parent signature).
